Terms and conditions for Automatic Renewal Alliance BJJ.
Automatic renewal for subsequent months (with service fee approx 3.75%), unless cancelled with written notice, with notification coming 30 days in advance of the next billing date. Any tuition not received on time will be assessed a $25.00 penalty fee after (5 days). Returned checks will be assessed as a $25 penalty fee.
I understand that it is my responsibility to make sure my payment is received on time. Should my credit card, bank account, or other form of payment change, I will notify Alliance Jiu Jitsu at least 15 days prior to the next billing cycle to avoid any penalties.
No refunds shall be made for membership dues paid.
Membership Rules:
At Alliance Jiu- Jitsu Vail (PRO VENTURE 7 LLC) we believe in building a school and environment that is friendly and respectful to all, as such, we have some rules relating to hygiene as well as etiquette to support this belief. Members agree to follow the school rules in place in support of our community. Members fully agree to follow and abide by the rules of the school and violation of such rules is a breach of the terms and condition set herein and as such, could result in membership termination. Management reserves the right to terminate an individual’s membership at any time for any reason.
PARENT OR GUARDIAN OR LIAISON OF SUCH, OF A MINOR: I, as parent or guardian or liaison of the above-named minor, hereby agree to the terms and conditions set herein.

Waiver of Liability, Assumption of Risk, and Indemnity Agreement
Waiver:
In consideration of being permitted to participate in any activities associated with taking classes and or participating at Alliance Jiu-Jitsu Vail (PRO VENTURE 7 LLC) and the use of the premises, I, for myself, my heirs, personal representatives or assigns, do hereby release, waive, discharge, and covenant not to sue Alliance Jiu- Jitsu, its Managers, members, employees, agents and assigns from liability from any and all claims including those which result in personal injury, accidents or illnesses (including death), and property loss arising from, but not limited to, participation in any physical activities on the premises. I understand that there may be video photography and/or still images being recorded at this event or any event and during classes. I hereby release use of said images/video to the facilitators of this event, with no expectation of compensation, monetary or otherwise.
Assumption of Risks:
Participation in any physical activities carries with it certain inherent risks that cannot be eliminated regardless of the care taken to avoid injuries/damages. The specific risks vary from one activity to another, but the risks range from 1) minor injuries such as bruises, sprains, floor burns, and soreness to 2) major injuries such as pulled muscles, broken bones, and fractures; 3) Communicable diseases-contracting/spreading all types of viruses and bacteria.
I, have read the previous paragraphs and I know, understand, and appreciate these and other risks that are inherent in class activities. I hereby assert that my participation is voluntary and that I knowingly assume all such risks. I have had the opportunity to speak with a physician to determine whether I should participate in class activities.
Indemnification and Hold Harmless:
I also agree to indemnify and hold harmless Alliance Jiu- Jitsu Vail (PRO VENTURE 7 LLC), its Managers, members, heirs, employees, agents and assigns from any and all claims, actions, suits, procedures, costs, expenses, damages and liabilities, including attorney’s fees, brought as a result of my involvement in Jiu Jitsu class activities and on premises.
Acknowledgment of Understanding: I have read this waiver of liability, assumption of risk, and indemnity agreement, fully understand its terms, and understand that I am giving up substantial rights, including my right to sue. I acknowledge that I am signing the agreement freely and voluntarily and intend with my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law.
PARENT OR GUARDIAN OR LIAISON OF SUCH, OF A MINOR: I, as parent or guardian or liaison of the above-named minor, hereby give my permission for this child or ward to participate in the above-named activity, and further agree, individually and on behalf of this child or ward, to the terms as outlined herein.
By signing this contract, you have read and agreed to all terms and conditions.
